How to use this form:

• This form is for setting up a new journal on a DC institutional repository or multi-journal site. (For single journals, your consultant will provide the appropriate setup form.)
• Use this form in conjunction with the Design Customization Guide to request a custom journal design. 
• Once the form is completed, email the form to your consultant.
• This form may also be used for journal redesign requests.
